I am not being paid by the league, team owner, coach or any players. I agree that I am in good health and understand that I am participating in a VOLUNTARY full contact football league. I understand that my participation is AT MY OWN RISK. I agree that I am holding blameless the NLFA and my prospective team owners and any members in the cases of injury or death.
